I would contact the author of the poem and ask them for permission. Not sure about a price range, but I guess that would depend on the author as well. Who knows, they might be happy with just name credit. Another way of going about it is making a deal with the author, perhaps offer them a share in the finished song's copyright - should the song make any money.
